As PATIENT, I accept and explicitly consent the treatment by mdb laboratori d'anàlisis clíniques, S.A. (pressing the option: YES, I GIVE CONSENT) of my personal data, for the purpose of providing clinical analysis service and, furthermore, that in its capacity as responsible for the treatment it may:

a) Incorporate into the automated files existing in the laboratory all personal data provided by the patient, as well as all health related data obtained through clinical analysis, in order to be able to contact the patient, keep track of his or her medical history and provide him or her with data.

b) Transfer, exclusively to the patient's physician, or to the medical center in the cases in which this might be required, the results of the requested clinical analyses.

c) Incorporate on the web page of mdb laboratori d'anàlisis clíniques, S.A. (www.labmdb.com) the results of the clinical analyses performed, to which the patient will be able to have restricted access through a personalized password.

d) Send to the patient by e-mail or through its mobile application, using the personal data provided, information about new laboratory services, analytical tests offers, opening of new centers, newsletters, etc.

Therefore, the legitimacy for the treatment will consist, on the one hand, on the need to provide the patient with the clinical analysis service described in previous sections and, on the other hand, on the EXPLICIT CONSENT of the PATIENT, pressing the option: YES, I GIVE CONSENT while using our web/app.

As to the time period the information will be kept, in accordance with the provisions of article 12 of the Revised Text of Law 21/2000, of December 29, 2000, regarding information concerning the patient’s health and autonomy rights, and clinical documentation; the following documentation must be kept in the clinical record, together with the identification data of each patient, for at least 15 years from the date of discharge from each care process: informed consent forms, reports of complementary explorations, pathological anatomy reports, etc...

The documentation that integrates the clinical history not mentioned in the previous section must be kept for at least 5 years from the date of discharge of each care process.

The PATIENT may exercise the rights of access, rectification, cancellation, opposition, oblivion, limitation and portability by contacting mdb laboratori d'anàlisis clíniques, S.A., at the following postal address: carrer Urgell, 161, Baixos, C.P. 08036 de Barcelona or e-mail: mdb@labmdb.com. Likewise, the PATIENT may, at any time, withdraw his/her consent to the processing of his/her data through the same procedure. The PATIENT has the right to file a complaint before the Spanish Data Protection Agency through the channels and procedures established for this purpose on its official website www.agpd.es.
